Community Care, an exciting homecare licensed agency looking for a Field Nurse Supervisor to provide assistance with oversight on all home care cases located throughout White Plains and the surrounding regions. Visits include initial assessments at start of care visits, revisits for status changes, incidental visits and recertification.

• Develops, implements, evaluates, revises, and provides input to the client's plan of care to include the physician's plan of treatment, problem list, nursing care plan and the aide's plan of care. Provides care according to the plan of care.

• Actively coordinates the care of the client through communication and written documentation to the physician, case manager, and supervisor, other disciplines involved in the client's care, labs, equipment, or supplies, and community resources.

• Supervises ancillary staff involved in the client's plan of care.

• Provides nursing services, treatment, and diagnostic, preventive and rehabilitative procedure as prescribed by the client's physician and in accordance with the agency's policy and procedure.

QUALIFICATIONS:

• RN Graduate of an accredited school of nursing and is currently registered with the New York State Board of Nursing

• 1 years experience in HOME CARE EXPERIENCE IS REQUIRED.

• Ability to work under pressure, ability to multi-task, have a flexible schedule, and the ability to communicate effectively.
